Client Agreement READ AND AGREE (ESSENTIAL FOR BOOKING) * The purpose of our business is to provide additional pregnancy scans. Our scans should not replace routine NHS scans. We provide self-elected, non-medical, non-screening and non-diagnostic scans only. ScanSanctuary does not screen for abnormalities and therefore if an abnormality is missed, ScanSanctuary cannot be held liable. Due to the non-diagnostic nature of the scan, we cannot rule out ectopic pregnancy. The only scan looking for fetal abnormalities is the conventional anomaly scan performed by the NHS at around 20 weeks. This is a 2D scan specifically used to look for any abnormalities and is still seen to be the best method in detecting problems. We want to make sure that you fully understand the scan you have chosen and how our clinic works. Please do not skip this agreement – it is really important that you read this before agreeing to have your scan with us. Please make sure that you have read and understood this document carefully, before agreeing to these terms and signing this consent form at the bottom. If there are any parts which you do not understand, please ask for assistance. It is really important to us that you understand all of it before giving your consent. When ‘ScanSanctuary’ or ‘we’ is mentioned, this refers to ScanSanctuary Ltd (registered in the Isle of Man number 133956C), scansanctuary.com or Jenifer Reid, Director. The Scans We Offer: • These scans should not replace your routine pregnancy scans. • We provide self-elected (voluntary), non-medical, non-screening and non-diagnostic scans only. • ScanSanctuary does not screen for abnormalities and therefore if an abnormality is missed, ScanSanctuary cannot be held liable. Even though our aim is to help our clients as much as possible, only a registered medical facility, such as a hospital can help you with abnormalities. • Due to the non-diagnostic nature of the scan, we cannot rule out ectopic pregnancy. • The only scan looking for foetal abnormalities is the conventional anomaly scan performed by the NHS at around 20 weeks. This is a 2D scan specifically used to look for any abnormalities and is still seen to be the best method in detecting problems. • If anything concerns the Sonographer during the scan, then it will be communicated to you that you should go to your normal hospital and it is your responsibility to do this. • ScanSanctuary’s sonographer will explain their findings to you based on basic observation, but any concerns must be confirmed by the NHS before any action is taken. It is your responsibility to ensure that you then consult with your NHS care provider. • We aim to complete your scan abdominally but if you have ordered an ‘early scan’ (from 7 weeks) we may need to undertake an internal (Trans Vaginal) scan. The sonographer will discuss this with you beforehand. We will ensure your dignity as much as possible and can provide a trained female chaperone in all of our scans, however the scan may have to be arranged for a different day. You can decline an internal scan, however if you do, you accept that we may not be able to provide you with conclusive information. Gender Identification • Gender / Sex identification cannot be 100% guaranteed. • It is only the opinion of the Sonographer and therefore should be taken as such. • ScanSanctuary accepts no liability for any incorrect indication of your baby’s gender. • If you do not wish to know the gender of your baby, the sonographer will attempt to hide the gender as much as possible, but if the gender is accidentally revealed, ScanSanctuary will not be held responsible. Quality of Results / Images • Due to the nature of ultrasound, there are several factors which can affect the quality of your images. This can include, but is not limited to; above average BMI, empty bladder, excess adipose tissue, baby facing the wrong way, baby’s hands/feet/limbs over baby’s face, anterior placenta, reduced fluid around baby, multiple pregnancy, and changes to bowel habits/bowel gas, and other factors, which can all adversely affect your scan. • If any of these factors apply and we are unable to get a satisfactory image, we cannot be held responsible, and your scan will still have to be paid for. • We will always endeavour to do our best to obtain the images required on the day of your appointment however, if this is not possible we may be able to offer a ‘rescan’ appointment (subject to availability), possibly at no extra cost, and at the discretion of management. • When attending for 3D/4D ultrasound it is important to note that the scan is baby led and therefore images will vary greatly dependent on baby’s position. One re-scan is offered at the sonographer’s discretion. • Please do not go to the toilet immediately prior to your scan. It is important that all scans be commenced with a moderately full bladder. Going to the toilet immediately before your scan can affect the results and ScanSanctuary cannot be held responsible. Respect for Professionals. • All scans are undertaken by an experienced Midwife Sonographer. We ask that you respect this fact and understand that they will always utilise all their skills and experience to obtain the best possible images in any circumstances, even if those circumstances mean the images may not be of the quality you were hoping for. Please note • All scans are to be paid for before / at the beginning of the appointment. • We reserve the right to modify or improve our service without prior notice. • We do not allow any personal photography or video recordings to be carried out in the clinic. • You must be 18 years old or over; we cannot scan anyone under 18 years of age. • We shall not be responsible or liable for any injury, loss or damage to personal belongings of yours or any of your guests, whilst you are on our premises. • We are not liable for any business losses as we only provide the services for domestic and private use. • If we have to delay or amend the date or time of your appointment due to an event outside our control, then we will contact you as soon as possible to let you know and we will take steps to minimise the effect of the delay. As examples, this may be as a result of any technical issues, changes in law or regulatory requirements. Provided that we do this, we will not be liable for delays caused by the event. If there is a risk of substantial delay you may contact us to end the contract and receive a refund for any services you have paid for, but not received. 1. I UNDERSTAND THAT TICKING THE BOX BELOW, COMPLETING THE REQUESTED INFORMATION AND CLICKING THE 'SUBMIT' BUTTON IS THE SAME AS SIGNING MY NAME IN AGREEMENT. I AGREE / CONSENT TO THE ABOVE.
